If you have expended all of the Grant funds by the time you submit your Grant Application for a 2018-2019 Community Grant, you should submit the Grant Report Form included as Exhibit A of your Grant Agreement (download below) as part of your 2018-2019 Grant Application. If you have already submitted your Grant Report, thank you!

There are separate Grant Reports Forms based on the grant award amount (under $10,000, and $10,000 and above).
You will be asked to upload the completed Grant Report Form in the Online Application.

COMMUNITY GRANTS FUND
INFORMATION FOR 2017-2018 GRANTEES
STATUS REPORT FORM
If you have not yet expended all of the Grant funds by the time you submit your Grant Application for a 2018-2019 Community Grant, you should submit a Status Report Form (download below) as part of your 2018-2019 Grant Application.
Download and complete the Status Report Form.
You will be asked to upload the completed Status Report Form in the Online Application.

Recipients of a Community Grant are able to apply for funding for the same project or program for up to three consecutive years. After three years of consecutive funding for a particular project or program, the Grantee is required to wait at least one year before reapplying for funding for that project or program. Grantees can also choose to apply for funding for a different project/program in subsequent years.

To apply for a 2018-2019 Community Grant (for the same or different project/program), you must provide a status update on the 2017-2018 Grant in one of three ways:

You have already submitted your Grant Report Form to the Foundation for your 2017-2018 Community Grant.

You will submit your Grant Report Form for the 2017-2018 Community Grant to the Foundation with your application for a 2018-2019 Community Grant.
​​

You will submit a Status Report on the 2017-2018 Community Grant with your application for a 2018-2019 Community Grant.
​​
As a previous year Grantee, you will also be required to answer an additional narrative question in the Grant Application asking how this year’s proposal differs from last year’s proposal.
COMMUNITY GRANTS FUND
INFORMATION FOR 2023-2024 GRANTEES
Recipients of a Community Grant are able to apply for funding for the same program for up to three consecutive years. After three years of consecutive funding for a particular program, the Grantee is required to wait at least one year before reapplying for funding for that program. Grantees can also choose to apply for funding for a different program in subsequent years.

To apply for a 2024-2025 Community Grant (for the same or different program), you must provide an update on the 2023-2024 Grant (or an active grant from a previous year with an approved extension from SBCF) in one of three ways:

You have already submitted your Grant Report to the Foundation for your 2023-2024 Community Grant.

You will submit your Grant Report for the 2023-2024 Community Grant to the Foundation (all grant funds have been expended) with your Application for a 2024-2025 Community Grant.

You will submit a Status Report on the 2023-2024 Community Grant (grant funds have not been fully expended yet) with your Application for a 2024-2025 Community Grant.

As a previous year Grantee, you will also be required to answer an additional narrative question in the Grant Application asking how this year’s proposal differs from previously funded programs.

Note that a Statement of Revenues and Expenditures must be included as part of the Grant Report.

Download and complete the Grant Report Form as well as the Statement of Revenues and Expenditures Template. You will be asked to upload the completed Grant Report, including the Statement of Revenues and Expenditures, in the Online Application.

STATUS REPORT

A Grantee that has NOT YET expended all of the 2023-2024 Grant funds by the time it submits its Grant Application for a 2024-2025 Community Grant should submit a Status Report (download form below) as part of its 2024-2025 Grant Application. (This requirement also applies to a grantee that has not yet expended all of the grant funds from an active grant from a previous year with an approved extension from SBCF.)
